Access Humboldt provides DVD copies for a $10 fee. Please fill out this form to the best of your knowledge and include this form and a check made out to Access Humboldt.

Copy rights are subjected to the presenters' approval.

Our policies define a series as programming with a common theme, format, length and title that is
produced weekly, on alternating weeks, or monthly, or other regular intervals, as agreed upon between
the Presenter and Access Humboldt Programming Department. Each program in a series must contain
at least 75% new material. No more than 25% of the programs can be re-runs.

Humboldt residents can submit programs (live or pre-recorded) to be played on Access Humboldt TV channels and on our online Community Media Archive.

By completing and signing this form, you assume full responsibility for the program content pursuant to all applicable laws and statutes, and that your program complies with AH programming policies which are available for review online or at our offices.

You must complete and sign this form for us to schedule playback for your program. Applications submitted without signatures or other important information will not be considered.

Nonprofit and community service notices, and Government-submitted announcements can be submitted and displayed on Channels 10 or 12 Community Bulletin Boards.  read more »
